In accordance with Archdiocesan Safe Environment best practices, please complete this form to give your child permission to participate in online youth ministry events. You or your child will also need to complete an RSVP form for the specific event they are attending in order to be sent the link to join the event.

The form collects email addresses to confirm who is filling out the form, so it must be completed with a parent email.
