2020 Youth Leadership Conference on Asian and Pacific Islander Health Application Form

OVERVIEW
The 18th Annual Youth Leadership Conference on Asian and Pacific Islander Health will be held from Wednesday August 5 - Saturday August 8, 2020 at Stanford University in Palo Alto, California.  

100 High School students will be selected to connect with each other online
-Learn from industry experts about leadership, network, and make friends
-Compete in a Team Leadership Challenge and present their projects to judges
-Experience life at one of America's top ranking Universities

Submit this Online Application Form by Sunday, May 10, 2020, at 5:00 PM Pacific Standard Time.  

Note on Fee:
-This year's event will be remote/online and free of charge

APPLICATION SUMMARY
Please be prepared to include the following in this Online Application Form:

Required Sections:
-Your name and general information
-The name and contact information for an adult who can write a letter of recommendation on your behalf, such as a teacher, sports coach, or volunteer supervisor.  This cannot be a family member.  Please note we will contact your reference directly and they must complete the form we send them by Monday, May 18, 2020 at 5:00 PM PST.
-Two short essay questions: 1) Why do you want to attend this Conference and how does attending this Conference align with your goals and interests? (250 words max), and 2) What strengths, talents, skills, or experiences do you have, that you feel will be a contribution to the Conference? (250 words max)

Optional Sections:
-Resume
-InspirASIAN Scholarship Application.  This scholarship grant ($500) is provided by InspirASIAN Northern California, and will be awarded to Northern California High School graduating seniors with a minimum of 3.4 GPA, who are actively engaged in the community, are recognized as a leader amongst his/her peers, and will be attending a 2- or 4-year accredited community college or university in Fall 2020.
-Financial Aid (N/A).  Students may qualify for a reduced Conference fee of $150 if they can provide one of the following: a) Documentation that you receive or qualify for reduced-price or free school lunch, or b) Proof of Medi-Cal or Medicaid benefits.
